Statewide Transition Plan Changes – Comment by April 6

The Indiana Family and Social Services Administration is seeking public comments on the modified Statewide Transition Plan (STP). Home and Community-Based Services members, providers, and stakeholders are invited to give their input about the plan.

You can read the plan here. Comments are due by April 6, 2016.  Comments may be submitted by email to HCBSrulecomments@fssa.in.gov or mailed to:

Comments will be tracked and summarized. The summary of comments, in addition to a summary of modifications made in response to the public comments, will be added to the modified STP and submitted to the Centers for Medicare and Medicaid Services.
